To help ensure a smooth and efficient appointment, please bring the following:

Referral slip from your dentist (if applicable) and any recent X-rays
A list of current medications

If you have dental insurance, please bring your insurance card. If unavailable, be prepared to provide the subscriber name, ID number, group number, and insurance carrier’s phone number so we can assist with claim processing.

Do I need to bring X-rays?
If your dentist has taken recent X-rays, please have them forwarded to our office or bring them with you. Digital X-rays can be emailed. If additional imaging is needed, it can be completed during your visit.
